Your facility is already listed here. Here is what that means.
KnowYourCareHome publishes the published inspection record for every licensed facility in Texas, exactly as CMS and the state published it. Yours is on the site whether or not you have ever heard of us, because it is a public record. This page explains what is free, what you can add, and what no amount of money changes.
What is already on your page, free
- Your name, address, phone, licence or CMS number, beds, owner and management company, Medicaid and Medicare, and Alzheimer certification, all from the published file.
- Your inspection counts: deficiencies or violations, how many were serious, how many surveys, and the date of the last one. For nursing homes, the four CMS star ratings and any fines.
- For nursing homes, how you compare with your county by deficiencies per survey, which is the fair measure. A home surveyed ten times is not punished for being surveyed.
- Every correction recorded, shown next to the finding.
Three things that never change
- Paying never hides, softens or reorders a record. A paying facility’s violations appear exactly as every other facility’s do. If we would not do that for you, no family could trust the page about anyone.
- The paywall applies to everyone, including you. The full report costs the same $12 whether you own the facility or are choosing one.
- Paid placements are labelled, every time, on every page they appear. That is the law and it is also the point.
Two ways to add to your page
Tell your side, in your words, on the page families are already reading.
- Your response under any finding. The published record stays where it is; your words sit beneath it, labelled as yours. “The plan of correction was accepted on 14 May and the follow-up survey found it in place.”
- Photos and your own description above the record.
- A Book a tour button on your page, linked to your calendar or your phone.

- One of 3 Featured spots on your city page, in a labelled block above the list where families are choosing among dozens, plus turns on the county page and the home page.
- Nobody is permanently first. The order rotates every hour, so each Featured facility is on top an equal share of the time. When a city’s 3 spots are taken, the next owner waits for one to open.
- Your phone number on the list rows, so a family can call without opening your page.
- Book a tour on your card as well as your page.
Both are month to month and stop when you say so. Find your facility’s page, pick a tier in the box on the right, pay by card, and reply to the confirmation from an address at your business with your licence number; we confirm you run it before anything goes on the page, usually within a business day.
Something in your record wrong?
The record comes from CMS and the state; we cannot edit it and would not. If the published file is out of date, contact CMS or Texas HHSC Long-Term Care Regulatory and the correction will flow onto your page on the next refresh. If we have shown something incorrectly, email chris@securelayerhq.com and we will fix it the same day, no tier needed.
